RELEASE OF LIABILITY AND WAIVER OF CLAIM FORM 

FOR FISHMASTERS MARINA  

BY SIGNING THIS DOCUMENT YOU WILL WAIVE CERTAIN LEGAL RIGHTS, 

INCLUDING THE RIGHT TO SUE PLEASE READ CAREFULLY.

I am aware that being present at Fishmastes Boat Launch during the current water conditions of the River poses an inherent danger and carries a risk of personal injury and/or death. I agree that I have decided to take full responsibility, fully recognizing the risk, and I accept the potentially serious consequences, some of which are foreseeable and some of which are not. I understand that I assume all of the risks if I am injured, killed or incur any other loss or damage and I acknowledge the following: 

•  I know that the risks and dangers associated with launch a boat may be caused by my own actions or omissions. 

•  It is my responsibility to inspect the Boat Launch prior to launching that I undertake and if at any time I perceive an area to be unsafe, it is  my responsibility to cease use of the Boat Launch.  · 

•  I freely and voluntarily assume and accept all of these risks and hazards and agree that my use of the Boat Launch shall be entirely at my own risk and that when I choose to use the Boat Launch it will be regardless of these risks. 

•  I understand that the fishmasters bait and tackle may not be in any way responsible for my safety during my use of the Boat Launch. 

By reading  this release and waiver, I hereby for myself, my heirs, executors and administrators, waive any and all rights or claims for damages 1 may have against the  Fishmasters Marina  

District. I understand that I am waiving and releasing any rights I may have to sue or to make any subsequent claims against the FISHMASTERS MARINA, its  employees, agents, volunteers, or others for whom the FISHMASTERS MARINA may be legally responsible . 2025
